Go to room no. 3 for form submission along with below documents.
- MRP size photo ( white background ).
- Transcript photo-copies
- Payment Voucher
- Citizenship photocopy
They will stamp on the payment vouchers and give you back.

Receive your document
After 7 - 10 office days, go to room 3 for receiving your document along with previous payment vouchers.
NOTE: take your valid identity cards like citizenship along with you. Showing Photo in Mobile may work as well.
If you are taking on behalf of your friend, take your citizenship card photo-copy along with you.
